Below you will find links to forms that you will need to download, print and complete prior to your initial visit. Please be sure to read and complete all documents in full. To help expedite your waiting time in the office, you may fax your completed forms to us at 631-475-1588. However, if you prefer, you may bring the completed forms in with you to your appointment.

All new patients are required to complete the New Patient Information Forms prior to the initial office visit. Please print these forms and fax the completed forms to us at 631-475-1588.
If your injury is the result of a job related Workers' Compensation case, please print and complete the Workers' Compensation Information. Please fax all forms to 631-475-1588.
If your injury occurred as result of an automobile accident, please print and complete the No-Fault Information Forms in addition to the New Patient Information Forms. Please fax all forms to 631-475-1588.
If your condition involves your hand, please print and complete the Hand injury information forms above. Please fax all forms to 631-475-1588.
IMPORTANT NEW PATIENT INFORMATION
Please bring the following when you come for your first visit:
- All prior medical information including x-rays, CAT scan films, MRIs and the accompanying medical reports .
We will need to see the actual films (disks are acceptable).
- All forms must be completed prior to your visit. Please complete them at home and fax them to us at 631-475-1588
to help expedite the waiting time in our office. If you are unable to complete the forms, please bring them in
to us 30 minutes prior to your scheduled appointment so that we may assist you. Failure to complete the forms
by your scheduled appointment time may result in your appointment being rescheduled.
- You must bring your health insurance cards with you. We will need to scan the cards into our computer system.
- If your insurance carrier requires a referral from your primary care physician, please call that physician
for the referral number or bring your paper referral. If you do not have this number or paper referral, you will
be responsible for the bill at the time of your visit or you may be rescheduled.
- If your visit is due to an accident, such as a Workers' Compensation or No Fault claim, it is imperative that
we have all insurance information prior to your appointment. If you do not provide this information, you will
be responsible for payment at the time of your visit.
- All fees and co-payments are due at the time of service. Please refer to your insurance plan or contract for information on what charges are covered. Any charges that are not covered are the patient's responsibility and are payable at the time of service. Payments are acceptable in cash or by check or credit card. Credit cards accepted here are:

Please arrive 15 minutes prior to your appointment to complete any additional necessary paperwork that was not faxed to our office.
If you are unable to keep your appointment, please notify us as soon as possible at 631-475-1224, option 2, so we may reschedule your appointment as well as accommodate another patient.
